Download the player waiver release form, fill it out and return it to the David’s Community Bible Church office at 1072 State Route 25, Millersburg, PA 17061.
Download the medical release form, fill it out and also return it to the church office.
Pay the $10 registration fee below.
Registration is $10 which covers game participation, team t-shirts and payment for the referees. Female students may register by clicking the “Pay Now” button below. You will be directed to the PayPal.com website.
If you wish to pay the $10 with a credit card, click the “Pay Now” button below which will take you to the PayPal.com website. Then, on the PayPal site, be sure to choose “Pay with Debit or Credit Card.”
If you wish to pay the $10 by cash or check, please drop payment off at the church office. (Make checks out to DCBC.)
Be sure to print out your receipt from the PayPal website and bring with you as proof of registration.
